Consent to the processing of personal data

Hereby, in accordance with Federal Law No. 152-FZ “On Personal Data” dated July 27, 2006, freely, by my will and in my interest, I express my unconditional consent to the processing of my personal data by MEDICAL VISUALIZATION SYSTEM LLC (TIN 7813214820), registered in in accordance with the legislation of the Russian Federation at the address:

197110, Russia, ST. PETERSBURG, LEVASHOVSKY, BUILDING 12, LIT. A ROOM 7 ROOMS 29H (hereinafter referred to as the Operator).

Personal data – any information relating to an individual identified or determined on the basis of such information.

This Consent has been issued by me for the processing of the following personal data:

– Name;

– Surname;

– Telephone;

– E-mail.

Data collection using cookies and other web analytics tools
Cookies are small files that are created and saved by the browser when visiting the websites of MEDICAL VISUAL SYSTEMS Ltd. Cookies are stored on the device for no more than 12 months and allow you to monitor the quality of the website and the characteristics of its use, as well as optimize marketing activities on the Internet.

If I withdraw my consent to the processing of personal data, the Operator has the right to continue processing personal data without my consent if there are grounds specified in clauses 2-11 of part 1 of article 6, part 2 of article 10 and part 2 of article 11 of Federal Law No. 152-FZ “On personal data” dated July 27, 2006
